Global billing for maternity is ending.

Today, maternity care is largely billed using a "global" code. Beginning January 1, 2027, CPT revisions will allow care to be reported more granularly at the service level. January 1 is the CPT effective date, but each payer might choose to transition billing on their own timeline.

If CMS finalizes the proposed 2027 maternity G-codes, UnitedHealthcare says commercial providers should report those G-codes. If CMS does not, dates of service on or after January 1, 2027 leave global obstetric reporting: 2026 services stay on CPT 2026, including 59425, 59426, or an E/M code, and 2027 antepartum and postpartum services use an E/M code, modifier TH, and an obstetric diagnosis. UnitedHealthcare says it may revise this if the final CMS rule differs. This does not replace the current global-package policy for dates before 2027.
