Global billing for maternity is ending.

Today, maternity care is largely billed using a "global" code. Beginning January 1, 2027, CPT revisions will allow care to be reported more granularly at the service level. January 1 is the CPT effective date, but each payer might choose to transition billing on their own timeline.

Transition: First antepartum visit 2026-05-15 through 2026-07-14 with delivery due Dec. 2026/Jan. 2027 uses 59426 plus delivery-only coding. The next cohort uses 59425 plus 2027 delivery-only coding. On/after 2026-09-01 for deliveries due after Apr. 2027, use E/M; 59425 may still apply when 4+ visits occur before 2027-01-01.

Global billing: The tip sheet retains defined 2026 antepartum visit-count codes during transition; service-level coding starts 2027-01-01.
