2027 Substrate Maternity Resource Center

Global billing for maternity is ending.

Today, maternity care is largely billed using a "global" code. Beginning January 1, 2027, CPT revisions will allow care to be reported more granularly at the service level. January 1 is the CPT effective date, but each payer might choose to transition billing on their own timeline.

Scope: Florida Blue notice also expressly names Truli for Health. It does not expressly identify Florida Blue Medicare products.

Transition: For a 2027 delivery, bill 1–3 2026 antepartum visits with E/M, 4–6 with 59425, and 7+ with 59426. From 2027-01-01, bill antepartum and postpartum E/M under CPT guidance; Florida Blue says TH is not required for routine prenatal E/M but is encouraged.

Global billing: For the stated 2027-delivery cohort, use the notice’s 2026 visit-count approach; global obstetric bundles end under the announced 2027 framework.

Florida Blue says affected contracts will be amended and a fuller 2027 policy is forthcoming. Do not use this notice for the separately listed Florida Blue Medicare record without a product-specific source.

Is global billing for maternity ending everywhere at once?

No. CPT revisions take effect January 1, 2027, but that is not each payer's billing date. Search tracked payers to see who published a transition date, whose guidance is still proposed, and whose policies are unchanged.
